This study explored horse-specific factors and management strategies linked to successful outcomes in horses with overriding dorsal spinous processes (ORDSP). Fifty-one horses were assessed via owner questionnaires. While 59% returned to riding, only 27.5% regained previous athletic function, and 17% maintained it for 12 months. Factors improving return to riding included higher training level, more weekly exercise, surgical treatment, completing rehabilitation, and saddle fitting. No factors significantly influenced long-term athletic maintenance. Surgical and structured rehabilitation approaches were most associated with positive outcomes.
